Agreements and Policies (Required)

REQUIRED FOR NEW AND CURRENT FAMILIES: This year, Walden students will be issued a device to support them with their classes and learning. K-2nd Grade students will receive an iPad and 3rd-12th Graders will receive a Chromebook. In order for your student to be given their device, you will need to fill out the technology permission forms available on the Family Information Forms page of the new FACTS Family Portal. If you filled out the forms last year, please access the forms to confirm that your information is still correct. If you do not have a family portal account yet, please see these instructions on setting that up and accessing it.

You will need to complete the documents for each student that you have here at Walden. These documents need to be filled out before you come to Walden to get your students' devices in the first week of August. Kindergarten and 3rd Grade students will receive their devices in class. For all other students, please use this link to sign up for a time to pick up your student's device.
